Can Long COVID Symptoms Come and Go?

September 16, 2026•5 min read

“Yesterday I felt almost normal. Today I feel awful.”

You know how it goes. You have a pretty good day, or even a good week. You get a few things done. You still pace yourself, but you can’t help wondering, “Does this mean I’m getting better? Is it smoother sailing now?”

Then you wake up and it feels like the Empire State Building just crashed down on you again.

All of the old symptoms come back. Or new ones pop up. Or you get a mixture of old and new.

And then the confusion sets in.

“What did I do wrong?”

“Is this something else?”

“Do I even have Long COVID?”

Unfortunately, this is all too common. Long COVID symptoms can come and go, and they can fluctuate considerably over time.

Symptoms don’t have to be constant to matter

I’ve said this before, but it bears repeating. No two people will have the same experience with Long COVID.

You might have the same symptom every day, but the intensity fluctuates. A symptom might disappear for days or weeks and then come back. Your symptoms might rotate as if they’re taking turns. New ones might crop up seemingly out of nowhere. Things can improve and then abruptly get worse.

A lot of people believe that any “real” medical issue has a distinct set of symptoms that are consistent.

But Long COVID isn’t like strep throat.

Strep throat primarily affects the throat and usually follows a fairly recognizable pattern. Symptoms such as a sudden sore throat, pain with swallowing, fever, and swollen lymph nodes tend to appear together. While every case isn't identical, there's a general pattern that makes sense to both patients and healthcare providers.

Long COVID doesn't work that way. The model is different.

Long COVID can affect multiple systems throughout the body, and those systems don’t always cause problems at the same time or in the same way from one day to the next.

Why symptoms may change from day to day

There are a few reasons why your symptoms can fluctuate so much. One is the balance between your capacity and the demands you place on yourself.

Your capacity depends on a number of factors, and how much physical and mental energy you expend is only part of it. Emotional stress, illness, sleep quality, sensory stimulation, and even the weather can all affect your capacity.

You don’t start every day with the same capacity. With everything that goes into determining it, your available capacity is going to be different from one day to the next.

So instead of asking only what you did that day to cause a symptom shift, ask yourself:

What has my body been dealing with lately?

And to make things even more complex, activity doesn’t always produce an immediate response. I’ve written before about Post Exertional Malaise (PEM) and how symptoms can sometimes worsen a day or so after the activity that triggered them.

That delay can make it even harder to connect what your body has been dealing with to how you feel today.

A good day is still a good day, but it isn’t necessarily your new baseline

I’m not telling you any of this to make you distrust those good days. Not by any means.

Enjoy the good days. Get a few things done.

But don’t try to get everything done. That’s just asking to crash later.

And don’t assume that a good day or two means your baseline has permanently improved.

Instead, treat those better days as another opportunity to learn about what your body can handle.

Become your Personal Health Detective

When you’re trying to figure out why your symptoms have changed, get curious about what’s been going on.

Instead of focusing only on what you did that day, take a step back and look at the bigger picture.

Ask yourself a few questions:

  • What happened in the day or two before my symptoms increased?

  • Was I more physically active than usual?

  • Did I use more mental energy?

  • How was my sleep?

  • Was there unusual emotional stress?

  • Was I dealing with more noise, activity, heat, or other sensory demands?

  • Have several demanding days accumulated?

You don’t have to obsessively track every single symptom. You don’t have to make detailed analyses of everything you do.

Look at the overall picture and pay attention to the patterns that start to appear.

Over time, you may start to recognize connections that weren’t obvious before. That information can help you make better decisions about how you use your available energy and capacity.

You don’t have to figure all of this out alone

Learning to recognize your patterns, understand your symptoms, and work with your available capacity takes practice. These are some of the skills I teach inside the Your Long COVID Transformation community.

We talk about things like pacing, energy awareness, symptom management, nervous system regulation, and practical ways to navigate everyday life with Long COVID or vaccine injury. Plus, you have a place to learn alongside other people who understand just how complicated all of this can be.

Changing symptoms don’t mean you’re imagining them

Long COVID doesn't always behave in a predictable or linear way. Symptoms can improve, worsen, disappear, return, or change in intensity. And when they do, it can be incredibly frustrating and confusing.

You don’t need to be able to predict every symptom or figure out exactly what caused every change.

Instead, keep learning about your body. Pay attention to your capacity. Notice the patterns that emerge. Use that information to make better-informed choices about what your body can handle right now.

That’s what being your Personal Health Detective is really about.

Your symptoms may feel random before you know what patterns you're looking for.
